Thanks for posting. For the Tie Bar Guide, see our 39-448. For the crank handle, see the 39-114L. If you need to replace the operator it attaches to as well, see our 39-188. Just make sure the dimension of the link arm below matches what you have. Let me know if it differs.

Looks like the one I have is actually 12 inches in length and the handle section is 5 inches.

Based on your new measurement of the hinge track, and reviewing your photos above, I actually think our 36-022 might be a better match. This is a 12" 4-bar style hinge. It may not be exactly the same as your original, but as long as you replace the top and bottom at the same time, and as long as the base track is 12", then you should be okay.
