I would recommend starting by comparing the 82-003 and 82-055 handle sets to yours, depending on the distance between the center of the installation holes in your original assembly. We'll include the dimensions from our product pages below. 

As for your patio door rollers, we may be able to help but we would need to see your original rollers removed from the door to compare to our inventory. Please also write out the housing height, width, and wheel diameter to help us narrow down your options. Thank you!
