Your operator resembles an older version of the 39-091 and 39-092 operators. Review the overall dimensions to confirm a good fit. For a new handle, take a look at the 39-230 or 39-224 fold-down style. Please note that the link arm that connects the operator to the window sash is discontinued and obsolete. You will need to reuse this arm for the window to function.
