At first sight, your balance resembles the Series 161 coil balance that is 1-1/4" wide by 9/16" thick. Please compare the dimensions to confirm a good fit! 

If you need assistance with choosing the correct weighted option, please provide the weight of the window sash to move forward. The sash will need to be removed and placed on a bathroom scale. 

Also review the 16-030 bracket, if needed.
