Milgard crank window crank assembly replacement

A home owner from Harpers Ferry, WV says:
Stamped on the assembly is "Truth" / Pat no. 4,840,075/ Made in USA. Also on one cogwheel is the number 3136 although I am not sure of the last digit. The house was built in 1996. Thank you for your help

Thanks for posting. This looks like our 39-217 to me. Review the diagram below and let me know if anything differs from your old operator, and if so, in what way. For a compatible cover and handle, see the 39-114L and 39-107, sold separately.
