Thanks for writing in! Your balance appears to be the S770-18 with a 62-105 Heavy Duty Hook attached. This balance is not a standard setup and will have to be custom made.

Before I can get the order link made up for you, I'll need to know the stamp from the metal channel. If there is no stamp, I'll need to know the exact weight of the sash (window). 

Also, I'll need to know the exact number of balances you are looking to order. There is an order minimum of (2) balances per custom order.

The compatible pivot shoe is part 15-105 but the pivot bar is discontinued and no longer made. You may be able to use the 26-042 as a substitute, but I cannot guarantee that it will work. Take a look at the dimensions for each part to determine if each of them are a fit for your window.
