Hello,

I can't be sure with the balance still installed but what I can see resembles the S795 channel balance. This assembly comes with the 1-1/4" wide 15-195 t-lock shoe but you will need to remove yours to confirm it's a match. It also uses a screw tab at the end of the pulley cord for installation. If yours uses a hook like the 15-198, just let us know.

I can't quite make out the stamp code on your metal channel but it should correspond to the strength option needed for your replacement balances. You'll also need to confirm the length of your metal channel, not including the t-lock shoe. Thank you!
