Hello! It's tough to make out what kind of top sash guide you have while it's still installed, but I think you might have a Pennco brand window. If I'm right, then our 18-060 and 18-061 are both possible options. 

Your next step should be to remove the original and compare its size and design to these tow products. That will allow you to pick the best option for the job, assuming that at least one of them is a match. If not, come back to me and we can work from there.
