Hello there! It looks like you could have an older Aldora/Arch commercial door. I recommend you take a look at the 79-121 pivot set. Review the specifications and see how they compare with yours. You may also want to look and see if there are any labels on the door, or even the key cylinder, stating the brand of the door.
