Take a look at our 39-315 and 39-316 as well as the 39-317 and 39-318. Look over the specifications and see if one matches up with yours. I can't make out the sash bracket from these photos but you can reuse yours if it is in working condition. 

If you need a new hinge, take a look at the 36-048 or 36-004-10. Either of these should work fine for you as long as you replace the top and bottom hinges at the same time.
