We'd be happy to look into this for you but we have not received any photos of your operator. If you could open the window and show us a clear top-down photo of the operator base, arms that move the sash, and hinge assembly, we can go from there. Thank you.

Your operator appears to be a match for our 39-188. We carry the 39-187 for the opposite side, along with the related items section displayed on both product pages. 

With that being said, these operators are rear-mounted, meaning that the screws install from outside of the window frame through the back of the housing. It looks like someone may have drilled screws through the front of your operator base for some reason, so I can't be sure if our replacements will work but take a look and see what you think.
