Hey Clay. This looks like the 39-031 EntryGard Dual Arm Operator to me. Just make sure the arm dimensions listed below match your old mechanism; let me know if anything differs. For a new cover and handle, see the 39-107 and 39-114L. For a new track, see the 39-505, and for a new stud bracket, see the 39-510. Let me know if you have any other questions.
