Hello there!

Thank you for reaching out to us. Our 39-187 and 39-188 operators look like a good match for what you show here. However, you will notice that the link arm for ours is 5". What you can do is drill out the old and new arms, then install the old arm on the new operator with the 62-014 rivet. To install this rivet, you'll have to hammer it in; I recommend doing it over a metal plate if you have one.

We don't sell a 4-bar hinge with your exact specifications but the 36-022HD has a 12" base. Take a look at the specifications and see what you think. You can also find our selection here
