The door locks that I ordered were the wrong ones. They were :(2) 10-561 : Lever Handle Assembly, ADA Compliant.
The one that is needed is a Globe ADA and the only number that could be found on it is 1. It is from about 1995 when it was installed at our church.

I'm afraid that information does not help us find what you need. Could you show us a couple pictures of your original latch?
