This looks like a match for our Series 795 channel balance with the 15-195B open cam shoe (sold separately). Compare our dimensions to yours to confirm that this matches up to your original shoe. Just be sure to triple check the metal channel length to ensure it is indeed 27". 

Your stamp "26-5" doesn't match any of our standard options so you'll need to weigh your sash on a scale to choose the appropriate stamp option. There's a chart on the S795's store page that will tell you which option you'll need based on the weight of your sash.
