This is from a Krestmark aluminum window from around 1990. I am a custom home builder but this is in my personal house.

Thanks for posting. At first glance, this looks either a Series 380 or Series 385 Channel Balance. The only difference between these two balances is the depth of the bottom nylon shoe, or the 'heel' to 'toe' measurement. See the diagram below for reference. Measure the depth of your old balance's shoe to determine which you need. Let me know if neither seems right.

To determine the length of the balance, measure the metal channel. Do not include the end attachments in that measurement. Finally you will need to choose a stamp option, which designates the strength of the balance. To choose a stamp option, look for a stamp on the metal channel.
