Channel balance replacement part

A home owner from Pompano Beach, FL says:

Window is approximately 24" high by 52" wide. It opens/closes up and down. I need to know the price and model for each channel balance piece.

Markings on the metal say,

Hello,

We'd be happy to help but it looks like your balance uses the 19-041 top fitting, which we can supply but it would need to be a custom order. We will also need to confirm the depth of the bottom fitting compared to the 15-004 and 15-005 shown below. 

Also, your stamp code indicates that you may have a 38-pound window sash but I would recommend confirming the exact weight since custom orders are not eligible to be returned. Confirming the sash weight would help us determine the exact spring strength needed for your replacement balances. Lastly, we'll need to know the number of balances you'd like to purchase, with the minimum being two. Thank you!
