Hey there, thanks for posting!  Your pivot shoe appears to be the 15-216 pivot shoe, which is compatible with the 26-402 side mount pivot bar. Look over the dimensions below to confirm that these are the right replacement parts for you.

Your balances will need to be a custom order as they have the 15-224 inverted link and the 15-198 hook attached to the Series 795 style channel. It will also have a 33F stamp for a 34 to 37lb sash.
