Our 39-253LB and 39-253RB appear to match what you have here. These operators are designed to use the 39-529 track slider (sold separately). The 39-545 and 39-546 brackets are sold separately as well if you need them. As for the hinge, we don't have an exact match as yours is a bit odd but the 36-048 is similar. Have a look and see if you think the specifications could work for you.

Note that you'll want to replace the top and bottom hinges at the same time.
