Thanks for posting. Looks like you have an S795-34 channel balance, but with a 15-198 hook at the top instead of a screw tab. I'm afraid this will need to be custom made. We can do it for you, but before we do, can you just tell me how many you need?

The pivot bar may be our 26-366, but it would depend on the style of the tip. It appears yours has broken off. Compare the 26-366 to an undamaged pivot bar in your window to see if they match.
