I'm not sure if we have an exact match for the lock in your photo, but it reminds me of one of our Escutcheon-style handles, such as the 38-205, 38-208, or 38-209. Take a look and compare the specifications of our parts to an intact lock assembly removed from the window to determine if one of these might be what you need. 

As for the folding handle and cover, take a look at the specifications on the 39-376 product page compared to yours. Note that the handle attaches to an 11/32" diameter operator spline. Thanks for posting!
