Thanks for reaching out! Your window operator reminds me of the 39-175 Sash Crank Operator. The opposite version is the 39-176. Take a look over the dimensions below to confirm that this is the correct match.

Unfortunately, we don to supply the replacements for the cover but the 39-109B will work to replacement the handle.
