Thank you for contacting us. Take a look at the 39-253LB and 39-253RB operators. These use the 39-529 track slider (sold separately). Review the specifications for these parts and see if they match your repair needs. 

Note that if the old cover can't be removed, you'll need the 39-451 and 39-452 covers/handles.
