This is the number on the hinge. I can't seem to locate one with the same offset and adjustment

Thanks for posting. I am afraid we do not have an exact match for this hinge. The trouble with many of these older cabinet hinges is that they are no longer made, so an exact replacement is often impossible to find. The 32-345 does seem to have a similar design. Take a look at it and see if you think it might work as a substitute.
