Thanks for the photos! Your balances can be replaced with either the S380-15 with a 1" shoe or the S385-15 with a 1-3/32" shoe. To determine which of these is the correct fit for your window, you'll need to confirm the measurement for the depth of the shoe.
Since your sash weighs 25.5lbs, this will need to be a custom made assembly.
Once you've confirmed the shoe depth, I'll be able to confirm pricing and availability for the balances you need.

I'm sorry to say the measurement you are showing in your photo is not accurate. The depth of the shoe is measured from the wing of the shoe to the back of the shoe. Take a look at the diagram below for reference.
If the measurement is over 1", you likely have the 1-3/32" shoe. Be sure to double check this before placing your order.
