Need replacement for a channel sash balance that is very similar to the 350 Series.
It is a 1/2" channel x 40" long. The printing on the channel is "39-15 CP5 CMC 1", and the spring (coil inside a coil) is color coded with an Orange dot and a White dot.
The top is very similar to your 19-002, per given dimensions, unsure of complete visual approval.
The bottom is close to your 15-004 shoe. The heel to toe is 1-3/32" but my wing span is only 11/16" instead of the 15/16".
The double pane window aluminum sash size is 31-1/2" wide x 42" tall.
There are two removal clips at the frame sash guide for each sash side. This might be an indication there is an option for 2 Channel balances, providing the need for the 11/16" shoe wing dimension.

Hello! We can supply this balance, but it will have to be a custom order due to its unique configuration. The Series 350 cannot replace this, nor can any other standard product in our inventory.
What I'll do now is email you a purchase link and price quote that you can use to add 2 of these to your cart. You can then checkout as normal. These balances come premade and are ready to install out of the box.
