See either our S351-16 or S390-16. The only difference between these two balances is the depth of the bottom nylon shoe, so compare both with your old hardware to see what matches best. I believe your stamp code might read 1520, which means you'd need a stamp option of 1520, which is for a window sash in the weight range of 6 through 10 lbs. See if that seems right to you.
