Markings on the back plate are RE28786 PAT and an image of an indented triangle pointing toward the single screw hole.
Screw holes on back plate are 21/16 and 1-5/16 from the cylinder.

Please see our 32-101. I think this may be a newer version of your existing hinge. The issue is, the manufacturer shortened the design of the tab to save on costs. The 32-101 may still work, but the installation holes on the tab are in different positions. You would need to drill new holes to match the hole setting.
