Hello,

We've seen this Miami box operator in the past and we do not believe that an exact replacement is still available. Our best recommendation would be the 39-054 mentioned in another customer's thread here with a similar assembly to the one in your photos. 

Take a look and see if you think that this might work for your repair but keep in mind that, unfortunately, the left-handed version of this operator was discontinued and is no longer available. Thanks for posting!
