Thanks for writing in! Your channel balance can be replaced with our S795-22. It comes assembled with a 15-195 1-1/4" T-Lock shoe like the one you have attached to your balance. 

You'll need to look at the metal channel to confirm the stamp from the balance. If there isn't a stamp, you'll need to obtain the exact weight of your sash; there is a chart below the product description the lists the weight range each stamp can support.

Also, your balance should have either a screw-in clip or a hook attached at the top. If it is a hook, there is a chance you may be able to use the 26-200 Balance Screw Plug, but that will depend on the shape of the installation hole at the top of the window channel. If it is not square in shape, you'll need a custom assembly.
