I am replacing the exact same looking roller with 3/4 inch wheel with 1/8 inch offset however at the top of the part it sez "NO 452" . Is this the same model? Also, the doors these are on are very heavy wooden doors. Would it help to put more than two rollers on the door? Or maybe a two-wheel roller of similar size?

Yes. So long as it has that 1/8" offset and a 3/4" wheel, the 23-031 seems like the best match to me.
