Hi, I couldn't figure out the style or series. it looked a lot like S385, but it could also be S380 or even S378.
It is as far as I can tell 9/16th inch deep. (1/2 inch).
Also I must have bent the takeout clips, so not sure if those are standard.

Thanks for posting! All of those series are viable options for the 26" balances you have here. To narrow down the correct replacement, you'll need to measure the depth of the bottom shoe. See the diagram below for reference.
You can likely use the 2540 stamp option but it would be a good idea to double check that your sash weighs between 19 to 25lbs.
We have a few different style take out clips. To be sure of the one needed for your repairs, I'll need to see a photo of the existing clip.
