Thank you for reaching out to us. We can help supply this balance but it will have to be a custom order due to its configuration. The pulley wheel is built into the bottom fitting itself and this balance features an extended cord travel as well as a metal channel length of 25-5/8" which is what makes it custom. Custom orders are nonrefundable so you may want to confirm to be safe.
I will just need a few more details so I can get this moving for you. Can you show us a photo of the bottom fitting from a side angle? Can you also confirm the number of balances you're looking to replace?

The part shown here is called a top sash guide but I'm afraid I can't ID it from this photo alone. Can you remove it from the window sash and show us "mugshot" photos of it, along with the dimensions?
Thanks! I recognize this guide now. I believe this is an old Milgard brand part. These unfortunately aren't made anymore, but we do have a modern alternative that we find works just as well. See our 18-009.
You'll note right away that this part has an extended edge at the top, which your original part lacks. This will have to be trimmed down with a utility knife to give it enough clearance to fit in your window. Otherwise, the dimensions should all match up. You should check them out just to be sure.
