Your balance looks like a match for the S770-28. Your stamp doesn't match our rating options, so you will have to weigh the window sash on a scale to determine the stamp for the new balances. There's a chart under the S770-28's product description that will tell you which stamp is best based on the weight of the window sash. 

Note that our Series 770 is designed to be installed with a screw. If your balance installs differently, please let us know. 

As for the shoe, the 15-167 comes to mind, but you will have to remove the old one to determine whether this matches up.
