I was able to narrow down your balance to two possible replacements. To determine which of the two is the correct one for your window, I'll need to see a photo of the bracket that connects the spring and the string. This is in the center of the balance.

Thanks for providing the additional photos. Your balance can be replaced with the S395-29 Channel. You can likely choose the 2830 stamp option but it may be a good idea double check that your sash weighs 16 to 23lbs.
