Thanks for posting. This looks like either an S351-29 or S390-29 Channel Balance. The only difference between these two balances is the 'depth' of the bottom nylon shoe, or the heel-to-toe measurement. See the diagram below for reference. Compare both balances with your old hardware to see what matches best. For a stamp option choose 2830.

I see that you had another post regarding the dimensions. Can you provide the exact length of the metal channel excluding the nylon attachments? If it is 28-1/4" or 28-3/4", unfortunately we cannot provide either of those lengths. Let us know!
