2830F need help ordering the channel balance top and bottom

I have window with 2830F Channel Balance. I am having trouble figuring out the correct parts to order.

The brace at the top of the sash that "catches" the channel balance have somehow gotten bent out of shape on one window (both sides) and the channel balances do not engage the window. I have 3 photos of the bent part and 2 photos of the same part from another window. The part is about 1/2" deep; 1-1/8" tall; and about 11/16" wide. Could you tell me which part to order so the channel balances will engage and catch on the sash.

Thank you for posting. For the top sash guide, check out our 18-013 or 18-012. I recommend you double check your measurements and see if either could work. It also looks like your nylon runner is damaged, so look at our 55-035.
