Thanks for posting! You'll need the S395-30 Channel Balance to replace yours. This has a winged top guide and wingless shoe like your existing assembly.

You can likely use the 2920 stamp option but I do suggest double checking to sash weight to confirm that it does fall between 12 to 16lbs.
