Based on the attachments, I can narrow it down to S385-30 or S380-30, depending on the size of the bottom shoe. Measure as shown below. For a 1" shoe, pick Series 380 and for a 1-3/32" shoe, choose Series 385. Your stamped "option" is outdated, so you need to weigh your window sash to be sure which option you need. There is a chart on the item webpage, in the product description, that tells you which option corresponds to which weight.
