I am confident we can replace this channel balance, but it appears to me the bottom fitting is damaged or has eroded. Is there an undamaged you can produce and show photos of? Once I see both fittings intact I can make a more definitive statement about what you need. For the Top Sash Guide, start by comparing an undamaged version to either the 18-009 or 18-166.
