31538 truth crank window identification

Question

2 arms, short arm says 31538, long arm is about 8 3/4"
There is a label, TRUTH and a phrase "Entry Gard"
There are 6 Phillips screws on the bottom to hold the unit down.

I need both a right and a left unit in white, and I need the whole kit---cover,handle, etc. In both the right and the left units, the crank will open the window, but if I try to close it, it seems like some threads are worn.
