There are a few different options. Could you show me a picture of the top nylon attachment? Your photos only show us the bottom one, which looks like our 1-3/32" deep 15-004. That's just an educated guess, though. Let me know if your shoe has a different depth.

As for the sash weight, can you confirm? Your stamp indicates a sash weight of 34 pounds, but you've given 41 pounds as the weight. We can custom make a balance that supports 41 pounds, I just want to be 100% sure that's what you need.
