39-089 with screw hole tab on the bottom

I am looking for a truth left hand dyer operator like pictured on this page except it has a third mounting hole centered on the bottom exposed.

I'm afraid we do not have this exact operator, with that mounting tab at the bottom. If you do need that support, you might be able to attach a small piece of aluminum angle or maybe a piece of wood trim at the bottom of our 39-089 base. Would that work for you?
