Andersen sash balancer

Have some very old, pre 1976, double hung Andersen windows and the sash cord broke on two of them. Can somebody identify the suitable part number for a replacement Sash Support Andersen Window Balance. It's approx. 6 1/4 x 2 3/4 with the following code - CC 4 25 85. Was also wondering if the window itself could be identified because I'll possibly be needing a jamb liner. Glass size of each pane is approx 24 1/2 W x 20 H. Attached pics might help. Thanks.
