Thanks for posting! This operator reminds me of the 39-264 Andersen Split Arm Crank. The link arm is attached to the 39-530 LH Split Arm Sash Bracket, the right handed version is the 39-531.

You'll find the compatible cover and crank handle tagged below this thread.

Carefully compare the specifications to confirm this is the correct fit.

Thank you, the base plate looks about 5".
I'll send a few more pictures with measuring tape.
Should I remove the trim to install?

Yes, the sill would have to be removed for the repair. Take a look at the video below. The featured operator is different, but the process for removing the wood sill will be the same.
