I am looking to replace parts for this crank handle. Couldn't find a model number - hoping you can help!

Hey there!

It looks like the worm gear came out with the handle. I'm afraid that can't be repaired or replaced, so you'll have to replace the whole operator. Can you show us a photo of the arms that move the window sash? That will help us narrow down some options.

Thank you for the additional photos! Take a look at the 39-147 operator. This resembles your old assembly but review the specifications and see if they match up.
