Your hardware looks like a good match for our 38-056 & 38-057 locks and 39-154 cover but we suggest comparing the dimensions to yours to confirm everything lines up. 

It appears your worm gear came out with your handle in your second picture. I'm afraid that can't be replaced or repaired so you'll need a new operator. We can help but we'll need to see a photo of your operator with the window open so that the arms that move the sash are fully visible.
