Help with tilt spiral balance pivot shoe

We'd be happy to look into this for you but we'll need a bit more information to get started. Is there another window with an undamaged version of this pivot shoe that you could show us? We'll also need you to write out the exact width and thickness of your shoe to narrow down your options.

If you need the spiral balances as well, we'll need to know the length of the housing tube and confirm if the diameter measures 3/8" or 5/8". Thank you! 

The pivot shoe shown in all three photos is missing the center cam that engages with the pivot bars in the sash. We'll need to see an intact version uninstalled, as well as have you write out the width and thickness, to determine if one of our replacements might work for you.
