Thanks for posting! Your casement window operator appears to be an exact match for the 39-253LB Encore Dual Arm Operator. The black nylon piece on the end of the straight arm is the 39-529 track slider, which is sold separately from the operator. If your existing cover is not removeable, you can use either the 39-375 folding cover kit or the 39-451 non-folding kit.

I've tagged the opposite handed versions of these parts as well as the compatible track below.
