Hey there,
Thanks for posting! Given the dimensions you've confirmed and the placement of the installation holes, I believe you may have the 39-350 Look/Rusco Awning Operator. Carefully review and compare it to your existing operator to confirm that this is the right fit.
The 33-070 5/16" Shoulder Screw is used for the end of the arm.
